Improving Healthy and Local Food Communications Among Health Care Providers and Patients

VT 2-1-1 maintains a comprehensive database of nutrition (and many other) resources avaible across the state. Several members of the Health Team will be meeting with VT 2-1-1 to explore partnering on creating specifc communications pieces that can be used by health care providers to promote nutrition and local food resources to patients.
