Description

The Nutrition Outreach Specialist will support our Nutrition Program, promote community nutrition 
programs, and help eligible individuals apply for 3SquaresVT. The Nutrition Outreach Specialist will help 
fulfill CVCOA’s vision of “A World Where Aging is Honored” and its mission to “Support Central 
Vermonters to Age with Dignity and Choice.”

Essential Functions

  • Provide phone-based information for 3SquaresVT.
  • Assist people with 3SquaresVT applications. Occasional home visits may occur to accommodate individual needs.
  • Develop an outreach plan and execute within our 3-county area.
  • Provide outreach materials to community partner organizations and table at events as needed.
  • Table at CVCOA sponsored events for outreach.
  • Work with CVCOA’s contracted registered dietitian to develop nutrition education events and support materials.
  • Plan and execute nutrition education events in community-based settings.
  • Enter and update all 3SquaresVT Nutrition Program client data into database.
  • Refer clients to Case Management, Information & Assistance, and/or Nutrition Counseling and other support programs for assistance as needed.
  • Complete client records and program reports as required.
  • Attend required meetings and training.
  • Provide 3SquaresVT information to case managers and other CVCOA staff.
  • Work with the Nutrition & Wellness Director to develop systems for data collection and reporting.
  • Support the Nutrition & Wellness department with the Home Delivered Meal program intakes and reassessments, including data entry.
  • Other duties as assigned by the Nutrition & Wellness Director. 


Knowledge, Skills, and Abilities

  • Associate degree or equivalent experience in human services.
  • Ability to understand the needs of older adults and to maintain client confidentiality.
  • Able to work well with people in a variety of situations in local communities
  • Attention to detail, well-organized, and able to collaborate and solve problems.
  • Knowledge of issues related to older adults and nutrition programs.
  • Effective written and interpersonal communication; experience with public speaking is helpful.
  • Knowledge of public benefits programs, including 3SquaresVT, is a plus.
  • Access to reliable transportation.
  • Access to reliable internet connection to work remotely.

    Computer Skills: 

  •  Office (Word, Excel, Outlook, Publisher & Power Point).
  • Experience with or ability to learn client management database.


Physical Demands

  • Able to travel within 3-county area in a variety of weather conditions.
  • Able to lift up to 25 pounds periodically (as necessary for outreach events.)
     

Mental Demands

  • Able to prioritize and multi-task.
  • Able to be self-motivated and to work independently without daily supervision.
  • Able to collaborate with a team.

Compensation and Benefits

Hourly Salary Range $22.00-$24.00 based on education and experience
